Preferred Medical Associaties and Bay Area Preferred Physicians,
traditional Medicare and Medicare Advantage Wellcare Duet. We are not
contracted with Anthem/Blue Cross PPO. Co-payments and payment for any
services not covered by a healthplan are due at the time of the service.
